If Golden Age appears to be a good fit, the resident and family can visit the home, meet the team, and see where their loved one would be living.

​We meet with the family and, whenever possible, the prospective resident to discuss health history, daily routines, mobility, personal-care needs, and the level of support required.
05. Create a Personalized Care Plan
Before moving in, we develop an individualized approach to care, including daily assistance and meal preferences or dietary needs.

Our team reviews the assessment to make sure Golden Age can safely provide the level of care the resident requires.
